Steps:

1. Use a plastic opening tool to pry up and remove the screw covers located in the corners by the speakers

2. Unscrew the four screws under the covers with a #00 screw driver.

3. Carefully pry open the front of the phone.
4. When opening the phone be careful of the ribbon holding the top to the rest of the phone.

5. Carefully pry off the screen.
